Payment Claim Form
Payment for a project can only be issued after the school has signed the payment claim form and the artist has ensured the form has been submitted to the PERFORM! office. Please use one form per project and allow two weeks from the date the form is SUBMITTED to the Coordinator for the processing of your payment. Payment claim forms must be signed by the attending teacher and returned by the artist to the PERFORM! office. Though PERFORM! will accept forms directly from the school, it is the responsibility of the artist to ensure that the form is submitted. PERFORM! cannot be responsible for whether the school submits the form so it is recommended that the artist personally submit the completed form to ensure prompt payment. Forms may be submitted by the following methods:

- Mail: PERFORM!, c/o Theatre Nova Scotia, 1113 Marginal Road, Halifax, NS, B3H 4P7
- Drop the form off to the office in person
